- Carry out the plan of care or support within the limits of own role
- Lead others to carry out their responsibilities when implementing a plan of care or support
- Adjust the plan in response to changing needs or circumstances

- Agree on methods for monitoring the way a care plan is delivered
- Collate and record monitoring information from agreed sources and agree on any changes
- Seek agreement with the individual and others about who should be involved in the review process and the criteria to judge the effectiveness of the care plan
- Seek feedback from the individual and others about whether the objectives of the plan have been met
- Work with the individual and others to agree on any revisions to the plan
