1.1 Demonstrate an in-depth knowledge of the condition your patient is living with.

You must address these subtopics in particular detail

1. a)    Explore the pathophysiology of your patient’s condition including causal factors such as genetics or lifestyle choices
2. b)    Discuss the signs and symptoms of the condition
3. c)   Discuss common medications used. You may wish to pick three medications, looking at their use, contraindications, common side effects, dosage, and anything else which would support/ impact your patient when taking them

Knowledge, Understanding, and Cognition

1. Demonstrate knowledge of pathophysiology, pharmacology, and treatment of common long-term conditions (e.g. dementia, cardiovascular and cardiac disease, chronic lung disease, renal and liver failure, diabetes, some cancers, chronic fatigue, chronic pain, Parkinson’s, and multiple sclerosis).
2. Describe and analyze the long-term support needs of people living with long-term conditions.
3. Evaluate the enhanced communication skills needed when breaking of bad news to people.
